The following describes our In-Home services:

Homecare is a state funded program that provides social and nutritional services in the homes of persons 60 years of age or older who have difficulties managing normal daily activities.  Services are designed to assist them in maintaining their independence, in their home, for as long as possible.  It provides the following in-home services based upon the needs of the individual.

  1. Assessment/Reassessment – The collection of in-depth information about a person’s situation and functioning.
  2. Case Management – A process for ensuring clients receive appropriate, comprehensive and timely services to meet their needs as identified by the assessment.
  3. Home Health Aide – Care provided by a licensed health professional subsequent to assessment and diagnosis of a physical, oral, mental problem or condition.  Includes rehabilitative care such as physical, speech/hearing and occupational therapies
  4. Homemaker Home Management – Provides assistance with light housekeeping, laundry, shopping and meal preparation.
  5. Personal Care – Provide s assistance with bathing, dressing, toileting, or other personal needs.
  6. Home Delivered Meals – A noonday meal delivered to a client’s home, hot or other appropriate meal that meets one-third of the Recommended Dietary Allowance (RDA).
  7. Respite – Supervision and care provided to a client to safeguard his rights and interest for the purpose of protection against harm to self or others.  The Caregiver is free to leave their home while relieved of caregiving responsibilities.

 

Eligibility: A person must be sixty years of age or older and conditions determined by a formal assessment and reassessment at least every six months.

 

Cost for Services: Cost is based on a sliding fee scale.  No one is ineligible due to excessive income, but they may be asked to pay more for services that a person with a lower income.
